Your Patient Portal
On your portal Home Page/Contact Info tab you can fill out your personal contact information and your partner’s. On the left hand side you can also view a list of all of your scheduled appointments 24/7.
ClientCare EMR
There are many benefits to your ClientCare EMR portal.
Under the Health History tab you will be able to enter in your “Dating and Menstrual History, Previous Pregnancies, Medical History, Social History, Family History, Allergies and Meds & Supplements.”
Under the Message Center tab you will be able to communicate with your midwife team. We will send the results of your lab work here, messages about common topics and you will be able to message us with questions.
Under the Forms & Records tab you will see sub tabs for “Consent Forms, Educational Forms, Medical Records and Uploaded Files.
“Consent Forms” is where you will find the informed consent documents for your signature.
“Educational Forms” is where you can view our library of information about topics such as other providers we refer to, nutrition & exercise, common complaints, third trimester help, birth education, baby education/medications, postpartum instructions, mood disorders, breastfeeding help and state required documents.
“Medical Records” is where you will be able to view your medical records. For example, you can click on the “labs” pdf and view your lab work, or the “labor records” pdf to view your labor flow.
“Uploaded Files” is where you can upload documents for your midwives such as previous medical history, ER reports, cesarean reports, etc.
Under the Financial Info tab you will be able to view your Financial Agreements (contract) and your “Payment Records” where you can access your invoice at any time.
Under the Birth Certificate tab you will be able to fill out the information needed to file your baby’s birth certificate with the state of Texas.
We suggest you save your username and password so you can log in easily.
